
TRANSITIONAL LIVING & RAPID REHOUSING
Our Transitional Living Program, Patti’s Place, currently consists of eight two-bedroom townhouses, designed for youth ages 18 to 24 who are experiencing literal homelessness. We can accommodate both individual youth and young families. Patti’s Place is a HUD-sponsored Youth Housing Demonstration Program facility aimed at providing housing for homeless youth in need of a safe environment. The program offers basic necessities such as housing and food, while they develop and learn essential independent living skills. Participants have the opportunity to work and save money as they engage in the program, preparing themselves for self-sufficiency.
A second component of the program is Rapid Rehousing, which supports youth in entering and maintaining their own leases. Youth who are ready for greater independence may qualify to move directly into their own leased property. Financial support continues for a designated period to ensure that the youth can sustain their household. This support includes rental assistance, food assistance, and case management services, all aimed at helping youth create a successful adulthood for themselves.
Both programs offer comprehensive case management services, life skills instruction, group and individual therapy, as well as assistance in finding employment. Participants are encouraged to engage in educational programs that may enhance their job prospects.
Eligibility, Enrollment, and Cost
Participation in the program is voluntary, and services are provided at no cost at Patti’s Place and sliding scale rent participation in Rapid Rehousing after a period of time. Youth must be between the ages of 18 and 24 to qualify for either program. A Case Manager will conduct an assessment interview with eligible candidates. Staff will provide information about the program and the requirements for participation if a vacancy is available. Youth may also be referred by other agencies.
