Building Bridges to Independence
Our YHDP (Youth Homelessness Demonstration Program) Transitional Housing Program provides safe, supportive, and temporary housing for youth and young adults experiencing homelessness. Designed for individuals aged 18–24, this program offers a stable living environment while helping participants work toward long-term independence.
Through individualized case management, life skills training, and access to education, employment, and health services, participants receive the tools and support they need to transition successfully into permanent housing. Our trauma-informed and youth-centered approach ensures that every young person is treated with dignity and empowered to take control of their future.
